Qualifications

  • CPS certification with 1 year paid or volunteer experience, or a High School diploma with 2 years in Human Services.
  • Excellent oral and written communication, detail-oriented, and able to document a high volume of information.

Responsibilities

  • Provide PSR services in facility-based and/or community settings as assigned.

Description

Purpose:

  • To provide Psychiatric Rehabilitation Services (PSR) facility based and/or in the community as assigned
  • To facilitate groups/units to enhance an individual's knowledge to develop, enhance and retain skills and competencies in living, learning, working and socializing.
  • To help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice.
  • To monitor and track progress of individual's in assigned caseload; participate in the development of individualized rehabilitation plans.

Supervision Received:

  • Receives direct supervision from the PRS Director/Team Leader or Psych Rehab Specialist if assigned.

Supervision Exercised:

  • N/A

Qualifications:

  • Bachelors degree, or Associates degree and 1 year work experience in mental health direct service, or
  • CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years work experience in Human Services, which must include 1 year of mental health direct service.
  • Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
  • Strong computer skills are required, as well as Internet research abilities.
  • Ability to work independently and as part of a team.
  • Knowledge of mental health system and supports.
  • Must possess a valid driver's license with an acceptable driving record. (preferred)
  • Must be able to use public transportation to meet with members out in the community or in their homes.

Scope:

  • Frequent talking and listening.
  • Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.
  • Must be available for local travel.
  • May require crisis intervention for emergency situations.
  • Participation in on call coverage as applicable.
  • May be required to report to work during emergencies including inclement weather.
