Description
Purpose:
- To provide Psychiatric Rehabilitation Services (PSR) facility based and/or in the community as assigned
- To facilitate groups/units to enhance an individual's knowledge to develop, enhance and retain skills and competencies in living, learning, working and socializing.
- To help individuals receive the level of and/or have access to the services/supports they choose, so that an individual can live in the environment of their choice.
- To monitor and track progress of individual's in assigned caseload; participate in the development of individualized rehabilitation plans.
Supervision Received:
- Receives direct supervision from the PRS Director/Team Leader or Psych Rehab Specialist if assigned.
Supervision Exercised:
Qualifications:
- Bachelors degree, or Associates degree and 1 year work experience in mental health direct service, or
- CPS certification and 1 year paid or volunteer experience, or a High School diploma/GED and 2 years work experience in Human Services, which must include 1 year of mental health direct service.
- Must have excellent oral and written communication skills, as well as be detail-oriented and able to complete a high volume of documentation in a fast-paced environment.
- Strong computer skills are required, as well as Internet research abilities.
- Ability to work independently and as part of a team.
- Knowledge of mental health system and supports.
- Must possess a valid driver's license with an acceptable driving record. (preferred)
- Must be able to use public transportation to meet with members out in the community or in their homes.
Scope:
- Frequent talking and listening.
- Requires lifting and/or carrying and the ability to fold, kneel, stack, stoop and bend which may include direct client intervention.
- Must be available for local travel.
- May require crisis intervention for emergency situations.
- Participation in on call coverage as applicable.
- May be required to report to work during emergencies including inclement weather.
