PURPOSE:
The Bronx Youth ACT program provides an integrated set of evidenced based treatment, rehabilitation, case-management and support services to children/youth residing in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches.Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.
POSITION OVERVIEW:
The Psychiatric Nurse Practitioner works as part of a multi-disciplinary team to provide treatment and support services to children/youth living in the community. The Psychiatric Nurse Practitioner is responsible for providing psychiatric services which includes the following: provision of a full behavioral health (includingmedicalhealth) assessment; diagnosis; treatment recommendations; treatment planning; clinical treatment; and follow up planning. The Psychiatric Nurse Practitioner participates in shared decision making with the client and multi-disciplinary team. S/he provides services to andmakes in person visits withchildren/youthand their families/custodians/caregiversin their homes and communities and collaborates closely with other service providers and systems with which the client interacts. The role will require some evening availability and rotating on-call coverage. The Bronx ACT Team Psychiatric Nurse Practitioner, inconjunction with the Director/Team leader, has clinical responsibility for monitoring recipient treatment and staff delivery of clinical services.
