Job description

Psychiatric Nurse Practitioner - Youth ACT Team

The Bronx Youth ACT program provides an integrated set of evidenced based treatment, rehabilitation, case-management and support services to children/youth residing in the community who are diagnosed with severe mental illness and whose needs have not been well met by more traditional service delivery approaches. Through a mobile, multi-disciplinary team of staff, recipients are supported in their recovery through an individualized, person-centered model of care.

POSITION OVERVIEW:

The Psychiatric Nurse Practitioner works as part of a multi-disciplinary team to provide treatment and support services to children/youth living in the community. The Psychiatric Nurse Practitioner is responsible for providing psychiatric services which includes the following: provision of a full behavioral health (including medical health) assessment; diagnosis; treatment recommendations; treatment planning; clinical treatment; and follow up planning. The Psychiatric Nurse Practitioner participates in shared decision making with the client and multi-disciplinary team. S/he provides services to and makes in person visits with children/youth and their families/custodians/caregivers in their homes and communities and collaborates closely with other service providers and systems with which the client interacts. The role will require some evening availability and rotating on-call coverage. The Bronx ACT Team Psychiatric Nurse Practitioner, in conjunction with the Director/Team leader, has clinical responsibility for monitoring recipient treatment and staff delivery of clinical services.

KEY ESSENTIAL FUNCTIONS:

  • Provide health/behavioral health assessment (including differential diagnoses, prognosis), treatment recommendations and treatment planning for clients across program types.
  • Provide client education on mental health issues and strategies for recovery and review patient’s progress and recommend discharge and aftercare planning.
  • Clinical responsibility for monitoring recipient treatment and staff delivery of clinical services
  • Prescribe psychotropic medication as appropriate for client care.
  • Attend mandatory YTAC trainings offered by the CTAC.
  • Provide medication management for clients.
  • Provide counseling, brief therapy, psychoeducation about health/behavioral health and other care for youth and families as planned by the multi-disciplinary treatment team and to support the multi-disciplinary treatment plan of the youth and families.
  • Attend daily team meetings and weekly one-on-one supervision.
  • Monitor, evaluate and record client progress with respect to care plan goals.
  • Provide on-call after hour crisis intervention services when needed to clients and families, and their support network.
  • Attend and participate in team meetings and supervisory sessions.
  • Participate in case reviews and contribute to treatment planning goal setting.
  • Provide consultations to non-medical staff on complex cases.
  • Participate in both the development and implementation of a person-centered care plan that includes formal and informal supports Communicate with health care providers outside of the agency and interpretation of health results for client care planning.
  • Engage clients and family as active partners in developing, reviewing, and modifying a course of care that supports the clients’ progress towards identified goals where appropriate.
  • Assess risk, develop safety plans and provide crisis intervention services.
  • Provide on-call after hour crisis intervention services when needed to participants and their support network, including respite referrals and other diversion and stabilization services.
  • As the ACT team requires 24/7 coverage by direct phone, the NPP will provide a portion of this coverage in rotation with other team members.
  • The NPP must complete an initial assessment visit in the community and visits in the community at least 80% of the time, including the required quarterly visits thereafter. If the recipient will not come to meet the NPP at the ACT office, the NPP must provide at least monthly services or as clinically indicated for that individual in the community.
  • Perform other related duties as assigned.

CORE COMPETENCIES for the position include:

  • Communicate practice knowledge effectively, both orally and in writing
  • Participate in agency quality improvement projects to ensure best client outcomes.
  • Create a climate of patient-centered care to include confidentiality, privacy, comfort, emotional support, mutual trust, and respect.
  • Work in supervision with central psychiatry team and directors to improve practice.

EDUCATIONAL/TRAINING REQUIRED:

  • Licensed as a Psychiatric Nurse Practitioner in the State of New York.
  • A current DEA registration for the prescribing of controlled substances. ANCC certification or pending application is required. NYS OPRA (Ordering/Prescribing/Referring/Attending) status approved or application pending required and a Health Commerce Account in place by the onset of employment. The former is needed to prescribe to clients with Medicaid, the latter is needed to adhere to NYS I-STOP law.

EXPERIENCE REQUIRED/LANGUAGE PREFERENCE:

  • Must be able to work as a team member with complex cases.
  • Working knowledge of Microsoft Office, ability to learn electronic health records and other software as required.
  • Fluency in Spanish preferred.

COMPUTER SKILLS REQUIRED:

  • Proficient with electronic health record
  • Microsoft Office (Word, PowerPoint, Excel, Outlook); comfortable and proficient using audio video platforms (i.e., Zoom).

VISUAL AND MANUAL DEXTERITY:

  • The work environment involves no physical risk or hazardous conditions.
  • To perform the essential functions of this job, the candidate is routinely required to sit (90% of the time) and stand (10% of the time)
