Job summary

Evergreen Health in Buffalo, NY seeks a Psychiatric Nurse Practitioner to provide psychiatric medication services and collaborate with a multidisciplinary team within an integrated care model.

The role requires NYS licensure, PNP certification, and current BLS/CPR. Travel between Evergreen locations is required; two years of direct PNP care is preferred. Schedule negotiable between 10-hour or 8-hour shifts.

Qualifications

  • Graduate of an accredited Psychiatric Nurse Practitioner training program.
  • Licensed to practice in New York State; certification as PNP.
  • Current BLS/CPR certifications.
  • Experience with EMR software (Medent preferred).
  • Two years of direct care practice experience as a PNP preferred.

Responsibilities

  • Screens patients for psychiatric, substance use, and co-morbidities and evaluates needs.
  • Completes admission and psychiatric assessments, histories, follow-ups, and required testing per regulations.
  • Prescribes and titrates medications; writes medical orders.
  • Coordinates care with internal and external providers in an integrated treatment model.
  • Attends case conferences, center meetings, and program initiatives.

Psychiatric Nurse Practitioner

Salary Range: $155,000.00 To $160,000.00 Annually

Psychiatric Nurse Practitioner is responsible for providing psychiatric medication services to patients, many of whom experience co-occurring mental health and substance use-related concerns. This position collaborates with and supports the multidisciplinary mental health team to ensure a patient-focused, harm reduction, and trauma-informed approach to care.

As part of the essential functions of this role, the Psychiatric Nurse Practitioner:

  • Screens patients for common psychiatric, substance use, and/or medical co-morbidities and evaluates co-existing conditions and the implications on the patient care/needs
  • Ensures the timely completion of admission assessments, required psychiatric assessments, psychiatric, substance use, and medical histories and follow-ups, lab testing, as required by state and federal regulations, accreditation standards, and sound medical practice
  • Prescribes and titrates the appropriate dosage of medications
  • Writes medical orders based on the individual needs of the patient
  • Works within an integrated treatment model coordinating treatment with both internal and external providers as indicated
  • Serves as a role model for positive, warm, welcoming, productive, and cooperative relationships with staff and clients
  • Adheres to Office of Mental Health (OMH) regulations and Evergreen Health policies and procedures, including regulatory timeframes on clinical documentation, using an electronic medical record system
  • Collaborates with and supports team members to ensure a patient-focused approach to care
  • Attends and participates in case conference, center meetings, and program initiatives

Qualified Candidate will be a graduate of an accredited Psychiatric Nurse Practitioner training program, be a certified Psychiatric Nurse Practitioner, and be licensed to practice in New York State; candidate must have current BLS/CPR certifications, and experience utilizing EMR software (Medent preferred); Two years of direct care practice experience as a PNP preferred; ability to travel between Evergreen locations is required; awareness of and sensitivity to the issues and health disparities faced by underserved populations is essential

Job Type: Full time; schedule is negotiable between 10 hour shifts or 8 hour shifts

Required Education, license/certification: graduate of an accredited Psychiatric Nurse Practitioner training program, licensed to practice in New York State; certified PNP, with current BLS/CPR certifications

Experience:Two years direct care practice experience as a PNP preferred
