Community Health Centers of America is seeking a Psychiatric Nurse Practitioner to work full time (5 days/week) or possibly part-time (3 days/week) at contracted skilled nursing facilities in the Burlingame and Concord areas with primarily in-person visits at nearby facilities and possibly telehealth appointments for other locations.

Qualifications

• CA licensed as PMHNP and NP
• Master's degree in nursing or related field
• 3–5 years of related experience preferred
• Current BLS/ACLS required
• Current DEA registration required

Responsabilités

• Provide direct psychiatric services including exams, diagnosis, treatment and follow-up in Skilled Nursing settings.
• Prescribe medications and manage psychiatric treatment plans.
• Educate patients about diagnoses, treatment and prognosis.
• Maintain complete and accurate medical records per established protocols.
• Coordinate care with nursing and medical staff and ensure safety.
• Monitor for adverse drug reactions and respond appropriately.
