PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER / HYBRID REMOTE

Riverside, CA

Salary: $160,000 - $185,000 + Bonus + Equity + Signing Bonus

At the direction of the Medical Director, the Psychiatric Mental Health Nurse Practitioner collaborates with PACE Physicians and the IDT team to provide assessments, planning, primary care, and ongoing medical education to participants.

The program enables seniors to continue living independently in their homes and communities.

Essential Job Duties :

• Provide psychiatric consultations, evaluations, counseling, and treatment in collaboration with PACE Physicians and Community-Based PACE Physicians.
• Perform in-person or telehealth psychiatric assessments with physicians and develop comprehensive care plans with the IDT team.
• Assist in discharge planning from acute or long-term care settings in coordination with physicians and the team.
• Refer participants to psychiatrists as needed, in consultation with physicians.
• Oversee care of participants in nursing homes, including regular visits based on participant needs.
• Serve as a consultant to the IDT, communicating participant changes, collaborating on care plans, and coordinating 24-hour care delivery.

Job Requirements :

• Master’s degree in nursing with a Psychiatric / Mental Health emphasis from an accredited institution.
• Certification as a Psychiatric Mental Health Nurse Practitioner.
• Active Psychiatric Mental Health Nurse Practitioner license in California.
• State CDS Certification and DEA License.
• NPI registration.
• Valid California Driver’s License with a clean DMV record.
• Recent vehicle insurance documentation.
• Minimum of two years' experience working with the frail or elderly in psychiatric mental health.

Must Have :

• Certification as a Psychiatric Mental Health Nurse Practitioner.
• Licensed in California.
• Active DEA registration.
