PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER / HYBRID REMOTE

Riverside, CA

160,000.00 - $185,000.00 + Bonus + Equity + Signing Bonus

Under the guidance of the Medical Director, the Psychiatric Mental Health Nurse Practitioner collaborates with PACE Physicians and the IDT team to provide assessment, planning, primary care, and continual medical education to participants.

The program aims to empower seniors to maintain their independence and live within their own homes and communities.

Primary Responsibilities :

• Conduct psychiatric consultations, evaluate psychiatric illnesses, provide counseling, and administer treatment to participants in coordination with PACE Physicians and Community-Based PACE Physicians
• Perform comprehensive psychiatric assessments in-person or via telehealth, along with the IDT team, and develop customized care plans for each participant
• Collaborate with physicians and the IDT in planning discharges from acute or long-term care facilities
• In consultation with the PACE Physician and / or Community-Based PACE Physician, make referrals to psychiatrists when necessary
• Oversee the care of participants in nursing homes in collaboration with the PACE Physician and / or Community-Based PACE Physician, and make regular visits based on participant needs
• Act as a consultant to the IDT, offering support and communication regarding participant changes, care planning arrangements, and coordination of 24-hour care delivery

Requirements :

• Hold a master's degree in nursing, specializing in Psychiatric / Mental Health, from an accredited college or university
• Be certified as a Psychiatric Mental Health Nurse Practitioner
• Hold an active unrestricted Psychiatric Mental Health Nurse Practitioner State License
• Have State CDS Certification and a DEA License
• Possess an NPI license
• Hold a valid State Driver's License with a clean DMV record
• Provide a copy of recent Vehicle Insurance
• Have a minimum of two (2) years of experience in Psychiatric Mental Health working with the elderly or frail

Must-Haves :

• Certification as a Psychiatric Mental Health Nurse Practitioner
• Active license in California
• Active DEA License
