PSYCHIATRIC MENTAL HEALTH NURSE PRACTITIONER / HYBRID REMOTE

Riverside, CA

Salary: $160,000.00 - $185,000.00 + Bonus + Equity + Signing Bonus

At the direction of the Medical Director, the Psychiatric Mental Health Nurse Practitioner works collaboratively with PACE Physicians and the IDT team to provide assessment, planning, primary care, and continuing medical education to participants.

The program enables seniors to continue living in their homes and communities.

Essential Job Duties :

1. Provide psychiatric consultation, illness evaluation, counseling, and treatment in collaboration with PACE Physicians and Community-Based PACE Physicians.
2. Perform in-person or telehealth psychiatric assessments in collaboration with a physician and develop comprehensive care plans with the IDT team.
3. Assist in discharge planning from acute or long-term care settings with physicians and the IDT.
4. Refer participants to psychiatrists as indicated, in consultation with the PACE team.
5. Oversee the care of participants in nursing homes, making regular visits based on participant needs.
6. Act as a consultant to the IDT, communicating participant changes, collaborating on care planning, and coordinating 24-hour care.

Job Requirements :

• Master’s degree in nursing with a Psychiatric / Mental Health emphasis from an accredited institution.
• Certified as a Psychiatric Mental Health Nurse Practitioner.
• Active State License without restrictions.
• State CDS Certification and DEA License.
• NPI license.
• Valid State Driver’s License with a clean DMV record.
• Recent Vehicle Insurance copy.
• Minimum of two (2) years of experience working with the frail or elderly in psychiatric mental health.

Must Have :

• Certification as a Psychiatric Mental Health Nurse Practitioner.
• Licensed in California.
• Active DEA license.
