Job summary

CommUnityCare Health Centers seeks a physician assistant or nurse practitioner to provide direct clinical care across specializations under physician supervision. You will perform physical assessments, order tests, diagnose and treat, and coordinate patient care within a federally qualified health center.

The role requires PA license or NP credential, with possible psychiatric mental health focus preferred. Excellent patient communication and adherence to regulatory standards are essential.

Responsibilities

• Perform comprehensive physical assessment using inspection, auscultation, palpation and percussion.
• Order or execute tests, analyses, and diagnostic imaging.
• Diagnose and treat diseases; refer to specialists as needed.
• Prescribe medications and therapies for treatment and prevention.
• Instruct patients and families on procedures, home care, and follow-up.
• Direct professional and ancillary staff during treatment.
• Respond to inquiries from staff and patients about preventive care and treatments.
• Compile medical data and record in EMR.
• Report deaths and disease outbreaks to authorities.
• Promote and coordinate high standards of medical practice.
• Demonstrate expertise in clinical practices and operations.
• Maintain computer/software proficiency for documentation.
• Work within credentialing privileges.
• Perform other duties as assigned.
