Job summary

Heritage Oaks Hospital in Sacramento, CA is seeking a Per Diem Assessment & Referral Registered Nurse (RN) to join the Admissions team. The RN coordinates admission processes, conducts nursing assessments, and ensures timely, courteous admissions in line with standards of care and hospital policy.

Qualifications include a nursing license in the state, with a Bachelor’s degree preferred and at least two years in psychiatric health care.

Qualifications

• Graduate from an accredited program of professional nursing.
• A Bachelor’s Degree is preferred.
• Two (2) years experience as a registered nurse in a psychiatric health care facility preferred.

Responsibilities

• Conducts and documents the Direct and Comprehensive assessments required for patients to determine dispositions.
• Participate in screening patients for admission into hospital programs, utilizing pre-approved admissions criteria.
• Reviews referrals and determines appropriateness for patient admission criteria with assistance of policy and procedures.
