Job summary

Columbus House, Inc. in Connecticut seeks a Permanent Supportive Housing Case Manager to assist adults experiencing homelessness in finding and maintaining housing, with linked medical, mental health, and substance use resources.

You will advocate for clients, maintain confidentiality, and coordinate with landlords and service providers. The role requires strong communication, organization, and documentation; 32 hours/week, Mon-Thu, 8:00am-4:00pm or 8:30am-4:30pm, with a salary of $25.50-$27.50

Qualifications

• Excellent organizational and time management skills.
• Strong written and oral communication in English.
• Proficiency with Microsoft Office and HMIS.
• Knowledge of housing resources and social services networks.
• Able to work with diverse populations.

Responsibilities

• Assist clients with finding, securing, and maintaining housing.
• Provide linkage to resources for clients at risk of eviction and for social, medical, mental health, educational, vocational, or entitlement needs.
• Communicate effectively with individuals served.
• Advocate for clients and support self-advocacy.
• Maintain boundaries and confidentiality.
• May provide transportation to appointments as needed.
• Coordinate with landlords and housing providers.
• Develop relationships with landlords to help clients move to community housing.
• Complete documentation and charting required by Columbus House, including HMIS entries.
• Attend meetings and provide coverage for vacancies as needed.
