Job summary

Columbus House, Inc. in New Haven, CT, is seeking a NOFO PSH Case Manager to deliver case management to adults experiencing homelessness living in long-term supportive housing.

You will assist with finding and maintaining housing, link clients to medical, mental health and substance use services, help with entitlements and job opportunities, and coordinate with community providers. Schedule is 32 hours per week (Tuesday–Friday, 8am–4pm or 8:30am–4:30pm), and the salary range is $25.50–$27.50 per

Qualifications

• Strong organizational and time management skills with a deadline-oriented mindset.
• Excellent written and verbal English communication abilities.
• Proficiency with HMIS or willingness to learn database systems.
• Familiarity with housing/homelessness services and community resources.
• Ability to build professional, respectful relationships with clients and providers.
• Knowledge of crisis intervention, confidentiality and harm-reduction principles.

Responsibilities

• Assist clients with finding, securing, and maintaining housing.
• Provide linkage to resources for eviction risk and access to social, mental health, educational, and employment services.
• Communicate effectively with individuals served and advocate for client needs.
• Coordinate with landlords, property managers and community agencies.
• Develop strength-based, person-centered service plans and document care activities.
• Attend regular meetings and provide coverage for vacant positions when needed.
