PRP Counselor Youth and Adults (Part Time) - Mental Health

Position Overview

The PRP Direct Care Counselor is responsible for assessing clients' needs and capabilities, providing supported living services, supportive counseling, skills enhancement, and crisis intervention to facilitate progress towards rehabilitation plan goals and community integration. The counselor works in community, home, and office settings with youth and adults experiencing mental health challenges. Services are individualized based on detailed client assessments, supporting continued career growth in the Social and Human Services field.

Responsibilities
  • Provide 4-6 direct service sessions per client monthly, as planned with the rehabilitation team and client.
  • Develop and update treatment plans (IRPs) every 3-6 months.
  • Assist clients in resolving problems, identifying resources, and promoting independence and self-advocacy.
  • Perform crisis interventions when necessary.
  • Maintain accurate client charts and documentation in accordance with agency policies and regulations.
  • Complete administrative tasks as required.
  • Perform other duties as assigned.
Expectations
  • Attend monthly supervisory meetings and trainings, including PRP team meetings.
  • Research and provide resources to clients.
  • Participate in agency-led groups/events at least once a month.
  • Notify supervisors of crisis situations promptly.
Qualifications
  • Ability to communicate with compassion and empathy.
  • Passion for impacting lives positively.
  • Excellent communication skills.
  • Sensitivity to cultural diversity and special needs populations.
  • Degree in a health-related field or 30 hours toward such a degree, OR one year of direct care experience in a supervised mental health setting with responsibility for IRPs.
