PRP Counselor Youth and Adults (Part Time) - Mental Health

Position Overview

Responsible for assessing clients' needs and capabilities, providing supported living services, supportive counseling, skills enhancement, and crisis intervention to facilitate progress towards rehabilitation plan goals and integration into community life. The PRP Direct Care Counselor works in community, home, and office settings with youth and adults suffering from mental health challenges. Services are individualized based on detailed client needs assessments. This role offers a pathway to advance your career in the Social and Human Services field.

Responsibilities
  • Provide 4-6 direct service sessions per client per month, as planned with the rehabilitation team and client.
  • Develop and review treatment plans (IRPs) every 3-6 months.
  • Help clients resolve problems, identify and utilize resources effectively.
  • Teach independence and self-advocacy skills.
  • Perform crisis intervention when necessary.
  • Maintain accurate client charts and documentation per agency policies and regulations.
  • Perform administrative tasks as required.
  • Other duties as assigned.
Expectations
  • Attend monthly meetings with supervisors and training sessions.
  • Participate in agency-led groups and events at least once a month.
  • Research and provide resources to clients.
  • Notify supervisors of crisis situations for decision-making support.
Qualifications
  • Ability to communicate with compassion and empathy.
  • Passion for impacting lives positively.
  • Excellent communication skills.
  • Sensitivity to cultural diversity and special needs populations.
  • Degree in a health-related field or 30 hours toward such a degree.
  • Or one year of direct care experience in a supervised mental health setting, responsible for minor IRPs.
