PRP Counselor Youth and Adults (Part Time) - Mental Health

Position Overview

Responsible for assessing clients' needs and capabilities, providing supported living services, supportive counseling, skills enhancement, and crisis intervention to facilitate progress towards rehabilitation plan goals and community integration. The PRP Direct Care Counselor works in community, home, and office settings with youth and adults suffering from mental health challenges. Services are individualized based on detailed client assessments. This role offers an opportunity to advance your career in the Social and Human Services field.

Responsibilities

• Provide 4-6 direct service sessions per client monthly, as planned with the rehabilitation team and client.
• Develop and review treatment plans (IRPs) every 3-6 months.
• Assist clients in resolving problems, identifying, and utilizing resources.
• Teach independence and self-advocacy skills.
• Perform crisis interventions when necessary.
• Maintain accurate client charts and documentation per agency policies and regulations.
• Complete required administrative tasks.
• Perform other duties as assigned.

Qualifications and Expectations

• Ability to communicate with compassion and empathy.

• Excellent written, interpersonal, and oral communication skills.
• Sensitivity to cultural diversity and the needs of the population served.
• Degree in a health-related field or 30 hours towards one, OR one year of direct care experience in a supervised mental health setting with responsibility for implementing activities outlined in IRPs.
