Job summary

You will work with clients, families, and healthcare professionals to develop treatment plans and document progress. Responsibilities include conducting assessments, providing guidance, monitoring goals, and maintaining thorough records.

Qualifications

• High school diploma
• Must be able to manage a caseload of 10 clients
• Strong leadership, organizational, and communication skills.
• Ability to work collaboratively across departments and manage multiple priorities.
• Proficiency in Microsoft Office and electronic health record (EHR) systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Coordinate and implement PRP services for individuals with mental health diagnoses
• Conduct assessments and develop individualized treatment plans
• Provide guidance and support to clients and families
• Monitor progress towards treatment goals
• Maintain accurate and up-to-date documentation of client progress and treatment outcomes
• Use personal vehicle to meet with clients in the community
• Conduct 100% of PRP sessions in person and within the community
• Contractors must maintain a minimum caseload of 10 clients
• For each adult client, contractors are required to conduct 6 visits per month
• For each child client, contractors are required to conduct 3 visits per month
