Job summary

New Day Health Systems in Catonsville, MD is seeking a passionate PRP Worker – Rehabilitation Coordinator to help individuals with mental health challenges live more independently. You will develop and implement rehabilitation plans, coach in daily living and community integration, and collaborate with treatment teams.

Strong interpersonal skills and independence are essential. A high school diploma is required; a bachelor's degree is preferred, and Maryland PRP familiarity is a plus.

Qualifications

• High school diploma or GED required; bachelor's degree in psychology, social work, or related field preferred.
• Prior experience working in psychiatric rehabilitation, mental health, or human services.
• Familiarity with Maryland PRP regulations and COMAR standards is a plus.
• Strong interpersonal and communication skills with a client-centered approach.
• Ability to work independently in community and home-based settings.
• Valid driver's license and reliable transportation required.
• CPR/First Aid certification or willingness to obtain upon hire.

Responsibilities

• Provide Psychiatric Rehabilitation Program (PRP) services to individuals with mental health diagnoses in community and home settings.
• Assist in developing and implementing individualized rehabilitation plans in collaboration with Rehab Specialist and clients and treatment teams.
• Coach clients on daily living skills, social skills, and community integration strategies.
• Monitor and document client progress toward rehabilitation goals in accordance with program requirements.
• Coordinate with case managers, therapists, and other service providers to ensure continuity of care.
• Transport and accompany clients to community-based activities and appointments as needed.
• Maintain accurate, timely documentation and progress notes in compliance with Maryland COMAR regulations.
