You will align with Intake, Operations, and Clinical teams, set KPIs, coach performance, and shape strategy to remove barriers and accelerate provider partnerships in a fast‑growing, patient‑centered organization.

Qualifications

• 5+ years in healthcare, with provider relations, healthcare sales/outreach, or referral management.
• 2+ years of people leadership experience.
• Experience with referral-based healthcare models (infusion, specialty care, etc.).
• Experience in infusion, specialty pharmacy, or outpatient services.
• Familiarity with HubSpot or referral tracking systems.
• Experience working cross-functionally with operations teams.
• Strong operator and analytical, performance-driven mindset.

Responsibilities

• Lead, coach, and develop a team of 10-15 Provider Liaisons.
• Set clear expectations, KPIs, and accountability around referral growth, provider engagement, and pipeline conversion.
• Conduct regular 1:1s and performance reviews.
• Build a strong, metric-driven, high-accountability culture.
• Oversee and optimize referral growth and patient conversion strategies across territories.
• Ensure strong relationships with referring providers and practices.
• Guide team on strategic outreach cadence and relationship-building best practices.
• Partner with leadership to identify high-value referral sources and expansion opportunities.
• Coordinate between Provider Liaisons and Intake/Scheduling/Authorization/Clinical teams.
• Identify and remove barriers that impact referrals and build no-referral-fall-through workflows.
• Lead service recovery efforts in escalated patient or provider concerns.
• Monitor referral pipeline performance and translation of data into team strategy.
• Partner on new market launches and service line expansion.
