Job summary

Vivant Health LLC is seeking a Provider Practice Support Advisor in the greater Sacramento area. You will serve as the primary liaison for an assigned portfolio of provider practices, coordinating resources to resolve provider needs and drive engagement with quality initiatives.

You will partner with departments across Quality, Risk Adjustment, Credentialing, and Operations to support onboarding, education, and issue resolution, while maintaining detailed provider documentation and strong

Qualifications

• Experience in provider relations, provider services, managed care, healthcare operations, or quality improvement preferred.
• Ability to coordinate multiple provider relationships and maintain accurate documentation.
• Strong written and verbal communication, interpersonal, and problem-solving skills.

Responsibilities

• Serve as primary relationship and coordination resource for assigned practices.
• Conduct provider meetings, outreach, education, and practice support activities.
• Support quality measures, care-gap closure, and health plan initiatives.
• Educate providers and office staff on health plan requirements, workflows, and systems.
• Coordinate onboarding, orientation, system access, and training.
• Route issues to internal departments and ensure timely follow-up.
