Job summary

U.S. Urology Partners seeks an Oncology Nurse Navigator to collaborate with Urology and Radiation Oncology teams for CRPC/Advanced Prostate Cancer patient care.

The role involves planning, referrals, and coordination of resources to meet individual patient needs and optimize care continuity. Responsibilities include implementing cancer care protocols, supporting quality metrics, and ensuring patients receive education and medications as prescribed.

Qualifications

• RN program completion from an accredited school.
• Current RN licensure required.
• Minimum of three years of clinical-setting experience, ideally in urology.

Responsibilities

• Coordinate patient care within CRPC/APC pathways to optimize outcomes.
• Facilitate referrals and coordinate resources with insurance and support services.
• Track patient progress, education, and medication adherence as prescribed.
• Collaborate with physicians, disease management teams and allied staff to ensure continuity of care.
• Assist with quality reporting, patient satisfaction, and treatment adherence metrics.
