Job summary

MedStar Health is seeking a Nurse Navigator for Urology to guide patients through diagnosis, treatment, and follow-up care. This role acts as a patient advocate and coordinates across urology providers, surgical teams, and support services to promote engagement and adherence to treatment plans.

The Nurse Navigator conducts assessments, provides education, facilitates referrals, and helps address barriers to care to improve outcomes and overall quality of care.

Qualifications

• Bachelor’s degree in nursing is required.
• Master’s degree in nursing is preferred.
• Active RN license and BLS certification are required.

Responsibilities

• Coordinate and guide patients through the urology care continuum.
• Provide education and support to patients and families.
• Facilitate referrals and monitor treatment adherence.
• Collaborate with multidisciplinary teams to ensure coordinated care.
• Maintain accurate documentation in the EHR and track outcomes.
• Support quality improvement initiatives and program development.
