Job summary

You will assess and plan care, educate patients and families, and collaborate with the care team. Massachusetts RN licensure and BLS required; BSN preferred.

Qualifications

• Massachusetts RN licensure required.
• 12 months to less than 3 years RN experience.
• Current Basic Life Support (BLS) certification.
• Department-specific certifications may be required.

Responsibilities

• Participates in and maintains awareness of performance improvement initiatives.
• Conscientiously evaluates and integrates evidence-based practice into clinical care.
• Conducts and documents accurate clinical assessments.
• Develops and evaluates the patient’s individualized plan of care.
• Uses nursing judgment to tailor the plan of care to patient needs.
• Develops educational plans for patients and families and documents per department standards.
• Supports informed decisions and advanced directives.
• Continuously evaluates and revises the care plan as patient status changes.
• Creates comprehensive discharge plans with referrals and resources.
• Precepts and supports new staff and unit transitions.
• Participates in professional development and departmental committees as needed.
