Qualifications

Responsibilities

• Perform routine physical/psychosocial assessments following the plan of care and notify physicians as appropriate.
• Formulate and participate in the plan of care for patient needs.
• Adapt educational content to patient and family with tailored teaching.
• Communicate care plans and changes to the interdisciplinary team and patient families.
• Make independent clinical decisions based on education and experience.
• Advocate for patient rights and resolve conflicts when needed.
