Qualifications

  • Requires Associate Degree in Nursing and current RN license.
  • 1-3 years recent acute care telemetry/critical care or step-down experience.
  • May consider 1 year continuous recent experience in Med/Surg if needed.
  • CA RN license required.
  • BLS CPR certification required.
  • ACLS certification required.

Responsibilities

  • Performs routine physical and psychosocial assessments.
  • Formulates an individualized plan of care and participates in planning transitions.
  • Adapts education to patient and family and integrates it into care.
  • Communicates plan and changes to the interdisciplinary team, patient, and family.
  • Makes independent clinical decisions based on knowledge and experience.
  • Advocates for patient rights and seeks resolution for conflicts.

  • Performs routine physical/psychosocial assessment to collect basic data: follows the written plan of care, standards, practice guidelines, utilizes pathways, which includes notifying physicians as appropriate.
  • Formulates a plan of care for individual needs, including patient population needs. Participates in the planning of routine transitional health care needs (i.e. treatment options, patient placement options).
  • Adapts planned educational programs and information to individual patients and family by modifying teaching strategies or content. Integrates education during the delivery of patient care.
  • Communicates patient's plan of care and significant changes to all members of the interdisciplinary team (using SBAR hands off reporting) and to the patient, family and significant others.
  • Makes independent clinical decisions based on knowledge drawn from education and experience.
  • Actively advocates for patient rights and identifies potential conflict. Seeks counsel for resolution of conflict.

Preferred:

  • Bachelors Of Nursing, upon hire and
  • Experience with A-line, CVP, and central monitoring., upon hire
  • Critical RN Adult/Neonatal/Pediatric/Acute, upon hire or
  • Progressive Care Certified Nurse, upon hire
