Job summary

Continuum of Care in New Haven seeks an Assistant Manager to oversee the DBT and David’s Path programs. You will assist the Program Manager with coordination, scheduling, and supervision of responsibilities within the assigned program, including training staff, interns, and volunteers, and providing hands-on support for daily living activities.

You will help plan and coordinate care, ensure quality service, manage medications following regulations, and support data collection and program

Qualifications

  • Bachelor’s degree preferred in a related field with at least 1 year supervisory experience.
  • Associate’s degree considered with minimum of two years’, or high school degree with at least three consecutive supervisory experience.
  • Prior experience working with people with mental illness, addiction disorders, and/or intellectual disabilities is required.
  • Valid Connecticut Driver’s license is required; reliable transportation to transport clients in personal vehicles if scattered site/non-24 hr program.
  • First Aid/CPR certification or willingness to obtain within 90 days of hire; maintain ongoing certification.
  • Proficient computer skills including Microsoft Office and electronic record keeping.

Responsibilities

  • Supports Program Manager in administration of program standards and procedures.
  • Provides face-to-face supportive assistance enabling clients to live in their apartment/home, including activities of daily living.
  • Ensures clients are treated with respect and dignity; provide quality care.
  • Coordinates client activities and supports client choices and decisions.
  • Administers medications following state and agency guidelines; trains staff as applicable.
  • Coordinates transportation for clients.
  • Supports interdisciplinary team in coordinating Individual Plans and Needs Assessments.
  • Tracks and reports daily data for behavior, goals, and ADL strategies.
  • Maintains electronic records and charting; ensures HIPAA compliance.
  • Ensures a clean, safe, and organized work environment for staff and clients.
  • Data input in designated electronic systems; submits monthly data per policy.
  • Maintains relationships with community providers and families.
  • Attends trainings, inservices, conferences to support professional growth.
  • Supports program changes reflecting agency priorities and strategic plan.
