Job summary

CAN Community Health Inc. in Phoenix, AZ is seeking a Program Specialist I to join the Education & Prevention Team. This entry-level role supports HIV prevention, testing, and treatment pathways with outreach in clinics and the community.

Requirements include a high school diploma and flexibility to work evenings/weekends; 1 year of related experience is preferred. We offer competitive pay, PTO, holidays, training, and comprehensive benefits.

Qualifications

High school diploma required.
Minimum 1 year of experience in a related field preferred; coursework may substitute.
Must be able to operate a motor vehicle and have valid insurance and driver's license.

Responsibilities

Provides HIV/STI/Hepatitis C testing and risk reduction counseling in community-based and clinic settings, including phlebotomy.
Educates clients on PrEP and links to PrEP programs.
Documents testing and services in electronic medical records and state reporting systems.
Develops partnerships with community organizations and establishes MOUs/BAAs under supervision.
Performs community outreach at shelters, bars, centers, and events; conducts testing and health education.
Distributes condoms and educational materials to clients and partners.
Maintains accurate records for prevention data collection and reporting.
Represents CAN Community Health at local, state, and national meetings and conferences.
Collaborates with internal and external teams to coordinate client services.
