BAYADA Home Health Care in Wilkes-Barre, PA is seeking an I/DD Program Manager to lead client-centered habilitation services and support independence for individuals with I/DD.

You will work with clients, families, and care teams to develop and implement ISP-aligned goals, supervise staff, and ensure quality care while navigating state requirements.

Qualifications

• Bachelor's degree required.
• Experience with the I/DD population.
• Prior supervisory or leadership experience.
• Ability to take on increasing responsibility and achieve goals.

Responsibilities

• Serve as an internal case manager and advocate for clients receiving habilitation services.
• Participate in Person-Centered Planning and support individualized goals.
• Collaborate with clients, families, case managers, habilitation technicians, and teams to implement and evaluate care plans.
• Conduct regular client visits to assess progress and ensure ISP compliance.
• Monitor and document progress toward individual goals and outcomes.
• Maintain communication with case managers, care coordinators, families, and stakeholders.
• Supervise, coach, and support habilitation technicians per policy and state rules.
• Participate in client-specific orientations and training.
• Support client intake activities and travel locally as needed.

I/DD Program Manager

BAYADA Home Health Care has an immediate opening for a I/DD Program Manager in our Wilkes-Barre, PA office.

As a Program Manager you will serve as an advocate and internal case manager for clients receiving habilitation services. You'll work closely with clients, families, care teams, and community partners to ensure individualized goals are supported while providing leadership and guidance to field staff.

What You'll Do

• Serve as an internal case manager and advocate for clients receiving habilitation services.
• Participate in Person-Centered Planning and support the development of individualized goals.
• Collaborate with clients, families, Case Managers, Habilitation Technicians, and interdisciplinary teams to implement and evaluate care plans.
• Conduct regular client visits to assess progress and ensure services align with the Individual Support Plan (ISP) and program requirements.
• Monitor and document progress toward individual goals and outcomes.
• Maintain strong communication with Case Managers, Care Coordinators, families, and other stakeholders.
• Supervise, coach, and support Habilitation Technicians in accordance with company policies and state regulations.
• Participate in client-specific orientations and training.
• Support client intake activities and travel locally as needed.
