We are seeking a Program Specialist to join our team.

Compensation: $28.75/hour.

The Program Specialist plays a critical role in the day-to-day management and oversight of specific client care within their identified Division. Reporting to the Program Director or Program Manager, this position is responsible for coordinating program activities and ensuring the delivery of high-quality services to program participants. This role may manage a caseload.

Job Functions of the Program Specialist:
  1. Coordinate individual care and services
  2. Facilitate team meetings as needed
  3. Coordinate healthcare and medication oversight
  4. Serve as the point person and liaison with family and stakeholder contacts
  5. Develop and implement service plans
  6. Work with internal and external parties to ensure unit authorizations in the ISP
  7. Aggregate data for quarterly reports and share with applicable parties
  8. Develop and coordinate HRST (PA requirement)
  9. Complete and coordinate annual consents
  10. Support Medicaid renewals and redeterminations
  11. Coordinate and facilitate Human Rights Meetings (NJ) or equivalent in other states
  12. Complete annual assessments
  13. Coordinate with the department on individual admissions, transfers, and discharges
  14. Compile individual-specific data for assessments
  15. Meet with Supports Coordinators (SC)/Case Managers to transfer ISP data
  16. Collaborate with Behavior Specialists regarding Behavior Support Plans
  17. Serve as a resource for the Program Manager to identify community activities for inclusion
  18. Participate in the development of Supports Intensity Scale (SIS)
  19. Analyze progress notes and data to verify ISP adherence and initiate changes as needed
  20. Ensure Peer Review recommendations are implemented
  21. Manage consumer burial reserve accounts with Fiscal Department
  22. Conduct compliance audits
  23. Collaborate with Nursing on healthcare issues and medication management
  24. Follow up on medical appointments, transportation, and refills
  25. Comply with incident management policies and procedures

Qualifications

The ideal candidate will possess the following qualifications:

  • Bachelor’s Degree or 60 credit hours in place of education
  • 3-5 years of experience
  • 1-3 years of supervisory/managerial experience
  • NADD Certification within 1 year of hire
  • Certified Technology Assessor required
  • HRST Reviewer required
