Essential Job Functions
  1. Assess active treatment strategies and make recommendations for needed services and interventions for residents.
  2. Lead the interdisciplinary team, coordinating and monitoring each resident’s Individualized Program Plan.
  3. Ensure consistency among external and internal programs and disciplines.
  4. Coordinate services, equipment, and programs to provide appropriate active treatment for residents.
  5. Develop and modify programs based on individual needs.
  6. Meet with team members to promote positive approaches and develop individualized intervention strategies.
  7. Consult with personnel from other agencies for evaluations and services.
  8. Participate in developing behavioral support plans with the behavior specialist and team.
  9. Participate in licensing inspections and maintain accurate progress notes.
  10. Assist with emergency psychiatric hospitalizations and incident management follow-up.
  11. Contribute to waiver submissions and participate in medication reviews.
  12. Write annual and quarterly program/service reviews and provide individual counseling as needed.
Other Job Functions
  1. Promote and protect the human and civil rights of individuals.
  2. Maintain a safe, clean, and positive work environment in accordance with Core Values.
  3. Adhere to confidentiality standards per HIPAA regulations.
  4. Uphold quality standards of practice and follow organizational policies.
  5. Participate in Incident Management activities and serve as a certified investigator as needed.
  6. Be available for the 24/7 On-Call rotation and perform other duties as assigned.
Knowledge/Skills/Abilities
  • Bachelor’s degree in a related field with at least two years of experience working with the IDD population.
  • Experience in writing social/emotional plans for individuals with low-frequency behaviors.
  • Successful completion of HRST rater training.
  • Detail-oriented with strong problem-solving, organizational, interpersonal, and communication skills.
  • Ability to pass background checks and hold a valid US Driver's License.
Supervisory Responsibilities

This position does not include direct supervisory responsibilities.

Reasoning Ability

Apply common sense to carry out instructions and solve problems involving multiple variables.

Computer Skills

Proficiency in Windows and Microsoft Office; willingness to learn agency-specific software (e.g., HCSIS, Enterprise Incident Management System).

Work Environment

Work occurs in various settings with moderate noise levels; physical crisis intervention may be required. Flexible hours are needed based on workload.

Physical Demands

Regularly lift/move up to 10 lbs, occasionally up to 50 lbs, and rarely over 100 lbs with assistance. Must have good vision and be able to stand, walk, sit, and use hands frequently.

Hazards

Low exposure to bloodborne pathogens and hazardous materials.
