The role you’ll contribute:

The Performance Program Manager plays a critical role in shaping infrastructure, processes, and technology that support provider performance management within our clinically integrated network and value-based enterprise. Reporting to the Executive Director of Provider Performance, this strategic role involves creating a sophisticated, proactive system that supports performance improvement initiatives, data-driven actions, and strategic planning across all providers in Florida, supporting the clinically integrated network and value-based enterprise.

The value you’ll bring to the team:

  • Performance Management Systems: Develop and maintain a robust system to manage provider performance, focusing on Quality Escalations, Utilization Escalations, Cost of Care Escalations, and Citizenship Escalations. Establish effective processes for Action Plan Management to ensure timely remediation strategies. Collaborate with executives and Medical Directors to establish provider accountability measures and integrate them into operational workflows.
  • Project Tracking and Collaboration: Implement and maintain a project tracking system for provider-facing teams to record Remediation Management and Accountability activities. Generate and implement strategies to enhance the performance management system as network needs evolve. Collaborate closely with Network Development and Network Governance to integrate these systems with network operations and with Network Operations leadership for system integration, clinical software, and CRM platforms.
  • CRM Architecture and Integration: Oversee CRM architecture (Salesforce) related to provider performance workflows in partnership with PHSO Operations. Ensure accurate records within the CRM to facilitate seamless team collaboration.
  • Performance Programs System Management: Lead initiatives related to Practice Transformation, Address Attribution Errors, and Incentive Management (validation, distribution, calculation, analytics, and collaboration with finance). Manage payer-specific performance programs and integrate applications like HFHP MA CDI Bonus Distribution, Curation Health, and Stellar Health into operational workflows.
  • Enhanced Analytics and Strategic Planning: Use advanced analytical skills and tools such as Microsoft Office, Tableau, and PowerBI, in collaboration with the analytics team, to create visualizations that impact performance management. Contribute to business analytics and strategic planning to support data-driven decision-making.

Qualifications

The expertise and experiences you’ll need to succeed:

  • Bachelor's degree in healthcare administration, Business, or a related field.
  • 4+ years of experience in healthcare.
  • Change Management Training.

Preferred qualifications:

  • Master's degree in healthcare administration, Business, or a related field.
  • Experience with data-driven performance management and strategic planning.
  • Familiarity with CRM architecture, provider incentives, and healthcare technology applications.
