Mental Health Partnerships (MHP) is seeking a Program Manager to provide overall service coordination of a Targeted Case Management team delivering flexible, community-based peer support to individuals with co-occurring disorders and homelessness.

This full-time role at 2709 N Broad St, Philadelphia, PA 19132 offers 40 hours weekly, a salary range of $56,000–$61,600, and potential for remote work one day per week after onboarding, with comprehensive benefits.

Qualifications

• Master's degree in a human services field or equivalent.
• 3+ years managing teams or programs in a health/social services setting.
• Strong understanding of clinical theory, community resources and systems.

Responsibilities

• Lead overall service coordination in line with regulatory standards and accreditation.
• Hire, train, evaluate and develop staff; supervise and schedule team.
• Foster relationships with external agencies and community organizations; seek referrals.
• Review electronic health records; ensure participant satisfaction and quality of records.
• Support participants with housing, mental health and substance use services per plan.
• Track program revenue and ensure staff meet productive unit targets.
• Manage safety, incident reporting and communications with leadership.
