Cap City Supported Living is seeking a Program Manager to lead Home Coordinators and Direct Support Professionals, ensuring that policies, procedures, and contracted services meet agency, state, and federal requirements.

The role emphasizes health and safety, effective resource use, and staff supervision for retention and continuity of care. The position includes on-call duties 24/7 and collaboration with QA to uphold service quality.

Formación

• Experience in supervising Home Coordinators and Direct Support Professionals.
• Familiarity with ISP contracts, BSP, and regulation compliance.
• Strong documentation and reporting skills.
• Ability to coordinate site team meetings.
• On-call availability 24/7.

Responsabilidades

• Lead and coach Home Coordinator Team and DSPs to ensure policy and contract compliance.
• Collaborate with Quality Assurance to maintain service quality and address deficiencies.
• Facilitate communication among staff and team members.
• Oversee staff retention programs and performance appraisals.
• Ensure staff training per agency guidelines and OAC requirements.
• Attend and coordinate regular Site Team Meetings.
• Review ISP/BSP and related services for individuals to ensure contract compliance.
• Maintain accurate records and complete required reports (HPCs, QAs, LPS).
• Respond to complaints, grievances and appeals; escalate as needed.
• Coordinate care among individuals, families, and support team.
• Ensure follow-up on QA and medical reviews; UI/MUI reviews.
