Responsibilities

  • Use NTBHA’s approved curriculum and assessments with clients according to abilities and mental health status.
  • Seek clinical consultation as needed from the Chief Clinical Officer.
  • Assist clients in crisis to access appropriate mental health services and continuity of care.
  • Provide timely reporting metrics to the Chief Clinical Officer or designee.
  • Make assessments and recommendations for clients’ mental health needs in compliance with laws and NTBHA policies.
  • Develop rapport with crisis-affected individuals using Motivational Interviewing and relevant methods.
  • Collaborate with state hospital staff, continuity of care staff, Medicaid MCO staff, peers, and family or legal representatives.
  • Prepare and maintain records, files, documents, and correspondence within NTBHA and partner agencies.
  • Work with NTBHA staff and partners to meet business needs.
  • Duties may be reassigned to meet organizational needs.
  • Bilingual ability preferred but not required.
  • Hours: Monday–Friday 8:30–5pm; flexible schedule as needed.

Position’s Role Summary:

North Texas Behavioral Health Authority (NTBHA), based in Dallas is responsible for developing a collaborative system of care for children, adolescents and adults with severe mental illness and other serious emotional disturbances, as well as those experiencing a substance use disorder. This position’s actions and work must align with NTBHA values and goals, regulations, and policies and procedures, and meet all documentation standards and regulatory requirements.

The State Hospital Step Down Program is designed to identify, assess, and facilitate the successful transitions of adults with serious mental illness (SMI), or a combination of SMI and medical needs exceeding the supports available in traditional settings who are clinically appropriate for transition from a hospital-based setting to a community-based services with proper supports.

The State Hospital Step Down Program Manager will oversee all aspects of the program including training, supervision, deliverables, and all roles listed for the manger in the Supervised Living Group Home Standards. The State Hospital Step Down Program Manager provides direct supervision to members of the State Hospital Step Down Program Care Coordinators or are part of the Intensive Transition Team (ITT) and oversees admissions into the program, including the residents of the Transitional Home as well as those residing in the community.

The Program Manager is responsible for ensuring the services delivered after enrollment, including oversight of the daily activities within the home. The ITT will plan for and complete the person-centered step-down/transition process. The ITT shall collaborate with, as applicable or determined necessary based on clinical presentation and client preference: state hospital staff, NTBHA’s continuity of care staff, Medicaid Managed Care Organization staff, a peer support specialist, a substance use counselor, the participant, and the participant’s family or legally authorized representative. The Program Manager represents the ITT in court-related processes including written reports and testifying, as approved by the Program Manager of State Hospital Step Down and/or Chief Clinical Officer.

Travel is required as part of this role’s duties. Travel can vary from 20% to 100% per day. This would include all counties within NTBHA Catchment area (Dallas, Ellis, Hunt, Kaufman, Navarro & Rockwall Counties). This role may be required to use a fleet vehicle and/or a personal vehicle for business needs. This may include, but is not limited to field & home visits, offsite meetings, co-located sites and/or pick up items for business needs, etc. If your program requires transport of individuals, this would only be allowed with a company vehicle and approval from management.

This position is classified as exempt and requires flexibility in scheduling to meet organizational needs, including adjusting work hours as necessary to ensure job responsibilities are completed.

QualificationsQualifications required:
  • Graduated from an accredited college or university with a master’s degree with a major specialization in Human/Behavioral/Social Services or related field.
  • Preferred: Licensed Practitioner of the Healing Arts (LPHA): LPC, LCSW, Licensed Psychologist, or LMFT and/or experience Qualified Mental Health Professional.
  • If a Qualified Mental Health Professional than a minimum of three years of experience in community mental health working with adult population with SMI and/or worked at a psychiatric hospital and/or Assertive Community Treatment (ACT) team.
  • Minimum of 1 year of supervisory experience.
  • Experience implementing new programs.
  • Familiarity with relevant laws and regulations governing mental health services.
  • Experience in working case-management.
  • Knowledge of trauma-informed care principles and strategies.
  • Experience in providing psychosocial education, skills training, or assessments, particularly with the target population.
  • Experience with participating in a treatment team that develops and monitors treatment plans for persons with severe persistent mental illness and substance use disorders.
  • Knowledge of the Texas crisis response requirements.
  • Knowledge of the various community resources and providers within the NTBHA service area including (but not limited to) supportive housing and employment, shelters, food banks, social service agencies, charities, medical and behavioral health providers, and other resources for the indigent population.
  • Possess and maintain a valid driver’s license. Pass and maintain a clear background record as required for healthcare organization under state and federal contracts in addition to requirements for working with the telehealth system as needed.
Job duties, responsibilities, and skills:
  • Use NTBHA’s approved curriculum and assessments with clients in accordance with their developmental and cognitive abilities as well as their mental health status and symptoms.
  • Seek clinical consultation as needed from the Chief Clinical Officer.
  • Assist clients while they are in crisis in accessing appropriate mental health services and facilitate appropriate continuity of care.
  • Provide accurate and timely reporting metrics to the Chief Clinical Officer or designee.
  • Make accurate and effective assessments and recommendations regarding the client’s mental health needs in compliance with the client’s rights as well as all relevant State and Federal statutes, and NTBHA policies and procedures.
  • Develop and maintain rapport with persons in crisis using modalities such as Motivational Interviewing and other relevant and appropriate methods in order to communicate and relate with callers in order to resolve crises.
  • Develop and maintain rapport with the various agencies and organizations involved with the State Hospital Step Down Program.
  • Prepare, create, and maintain records, files, documents, reports, and correspondence both within NTBHA and partner agencies and organizations.
  • Work with other NTBHA employees, members of partner agencies and organizations, as well as the target population and their families in a constructive and efficient fashion.
  • Duties and projects may be assigned or changed to meet business needs.
  • Bilingual ability preferred but not required.
  • Hours and shift assignments: Monday – Friday 8:30-5pm.
  • Position requires a flexible schedule if Program Manager is needed to cover program needs.
  • Must be able to work on and have knowledge of a PC (personal computer).
  • Advanced Microsoft Office Suite skills - Outlook, Word, Excel, PowerPoint, etc., with an ability to become familiar with company-specific programs and software.
