Job summary

Via Care Community Health Center seeks a Program Manager to oversee the daily operations of the JCOD Care Management Program in Los Angeles County. You will supervise Community Health Workers and staff, ensuring participant‑centered services across behavioral health, housing, and employment supports.

Leadership will drive outcomes and compliance with contract requirements. You will collaborate with internal departments, care plans, and community partners to strengthen service coordination,

Qualifications

Experience supervising community health staff and programs.
Ability to coordinate care management and service navigation.
Knowledge of HIPAA and confidentiality requirements.
Strong collaboration with partners across departments and providers.
Proven track record in program outcomes and quality improvement.
Excellent communication and coaching skills.

Responsibilities

Supervise Community Health Workers and program staff to ensure high-quality service delivery.
Provide onboarding, training, and ongoing professional development for staff.
Monitor participant engagement, outcomes, and program compliance.
Coordinate with care managers, providers, and community partners to support reentry and services.
Prepare program reports and support audits and site visits.
Identify gaps and drive quality improvement actions.
