Job summary

P3 Health Partners is seeking a Program Manager to oversee the day-to-day operation of the Red Rock Region’s admission and medical expense reduction program. You will collaborate with the VPMA, engagement managers, and care teams to review high-utilizer members, develop plans, and deliver them to primary care physicians to reduce avoidable admissions.

Qualifications

• Three years of experience in primary care.
• Two years in a managed care or population health environment.
• Experience coordinating care for complex, high-utilizing patients preferred.

Responsibilities

• Lead weekly IDT meetings end to end, including agenda, list, decisions and follow-through.
• Synthesize clinical records across affiliate EMR systems into a concise case summary.
• Translate IDT decisions into one-page clinical impressions and plans for primary care physicians.
