Job summary

1220 Homes for Independent Living of Wisconsin LLC is seeking a Service Coordinator (Program Manager) in Sheboygan to lead various programs, acting as the primary contact between guardians, clients, and our team. Candidates should have strong communication and organizational skills and the ability to work autonomously.

The ideal applicant should possess at least 3 years of direct care experience, preferably with supervisory experience, and have a valid driver's license. Benefits include health insurance, PTO after 60 days, and referral bonuses.

Qualifications

• At least 3 years of experience in direct care and 2 years of supervisory experience preferred.
• Valid Wisconsin driver’s license with 3 years of acceptable driving record.
• Personal vehicle with insurance.

Responsibilities

• Lead multiple programs, including employee operations.
• Interview and complete onboarding of new hires.
• Develop and manage individual service plans (ISP) for assigned clients.
• Ensure delivery of services outlined in ISP.
• Respond to program emergencies outside normal business hours.
