Job summary

Mypathcompanies is hiring a Service Coordinator in Sheboygan, Wisconsin, to lead multiple programs supporting clients with behavioral and medical disabilities. The coordinator will manage individual service plans, oversee household operations, and serve as the primary contact for clients and guardians.

Candidates should have a High School Diploma (Bachelor’s preferred), relevant experience, and strong communication skills. Benefits include health insurance, PTO, and a $2,000 referral bonus program.

Qualifications

• 3 years experience in direct care and 2 years supervisory experience preferred.
• Valid Wisconsin driver’s license with 3 years of acceptable driving record.
• Personal vehicle with insurance.

Responsibilities

• Lead multiple programs and oversee household operations.
• Interview, onboard, and train new hires.
• Develop and manage individual service plans (ISP) for assigned clients.
