Job summary

Community Care, Inc. is hiring a full-time Case Manager/Care Manager to join our Family Care Program in Sheboygan County. The role focuses on community-based case management, coordination with an RN Care Manager, and developing person-centered care plans.

The position offers a weekday schedule (Mon-Fri, 8:00 AM–4:30 PM) and a $5,000 sign-on bonus. Bachelor's degree in Human Services and relevant experience are required, with certification eligibility for long-term care screener.

Qualifications

• Bachelor’s degree in human services or closely related field required.
• Minimum of three years’ experience with seniors or adults with physical/intellectual disabilities; can substitute for non-human services degree.
• One year of experience with older adults or persons with disabilities preferred.
• Eligibility for long-term care functional screener certification required.

Responsibilities

• Manages an assigned caseload of program members with RN Care Manager collaboration.
• Completes comprehensive member assessments, care plans, and functional screens.
• Provides ongoing care coordination and quality assurance monitoring of services.
• Maintains required documentation in adherence to established standards.
