Job summary

Services for the UnderServed is seeking a Program Director to supervise all program staff responsible for Case Management for residents with mental health diagnoses, HIV/AIDS, and substance use histories. The role focuses on quality care and adherence to policies while guiding clinical teams.

The successful candidate will lead development and implementation of service plans, coordinate with staff, and ensure accurate reporting to leadership.

Qualifications

  • Master’s degree required.
  • Minimum of 3–5 years of supervisory experience.
  • Experience with HIV/AIDS population and mental health.

  • Ensure compliance with Federal, State and Local laws and regulations as they apply to the community residence program and agency policies.
  • Develop and implement individual client service plans in conjunction with clinical staff, counseling team and client.
  • Coordinate with Intake Coordinator for orientation and transitioning into program of all new residents.
  • Participate in discharge planning and provide emergency and relief coverage as necessary.

Position Overview
  • SCOPE OF ROLE :

The Program Director will provide supervision of all program staff with regard to Case Management to ensure quality care for residents with mental health diagnosis, HIV/AIDS, and histories of substance use.

  • ESSENTIAL DUTIES & RESPONSIBILITIES:
  • Familiar with Federal, State and Local laws and regulations as they apply to the community residence program and agency policies
  • Direct supervision of all program staff with regard to Case Management
  • Development and implementation of individual client service plans in conjunction with clinical staff, counseling team and client
  • The Program Director is responsible for ensuring that contracts with clients reflect goals of service plans for assisting counselors to develop intervention goals and activities which support the service plan
  • Coordinates Annual Functional Assessment Review for each client, submitting report and recommendations to Utilization Review Committee.
  • Responsible for maintaining contact with other service agencies and insuring client placement for treatment and program activities (may include volunteer job placement)
  • Responsible for coordination with Intake Coordinator of orientation and transitioning into program of all new residents
  • Participates in discharge planning
  • Provides emergency and relief coverage as necessary
  • May represent agency with community groups and agencies
  • Prepare report s for Assistant Deputy Director of Mental Health Services and Executive Director, including any problems, and proposals for changes in policies or program design
  • Provides supervisory meetings with all staff and maintains documentation of these meetings
  • Ability to work flexible shifts including weekends as assigned
  • Other duties as assigned including any problems, and proposals for changes in policies or program design
