Job summary

Crestwood Manor in Fremont, part of Crestwood Behavioral Health, is seeking a Program Counselor to provide direct care and support to residents in the Activity and Social Treatment Programs.

The role focuses on planning and delivering activities, promoting learning of skills for functioning, independence, and self-esteem, while meeting residents' spiritual, emotional, and psychosocial needs. Full-time schedule with competitive benefits.

Qualifications

• Minimum of a high school diploma/GED. Some college preferred.
• Experience providing activities or other group treatment preferred.

Responsibilities

• Plan, organize, and provide direct care to residents in the Activity and Social Treatment Programs.
• Ensure residents have ample opportunities to learn and practice skills for functioning, independence, self-care and self-esteem.
• Address residents' spiritual, emotional, recreational and psychosocial needs in accordance with policies and procedures.
• Follow federal, state and local standards and Crestwood policies in delivering care.

Job Title: Program Counselor

Job Duties: The Program Counselor is responsible for assisting in planning, organizing, and giving direct care to residents in the Activity Program and Social Treatment Program (STP) by current existing federal, state, and local standards, as well as our established policies and procedures. Responsible for ensuring that residents are given ample and regular opportunities to learn and practice skills for improving functioning, independence, awareness, self-care, and self-esteem. This role is also responsible for ensuring that residents' spiritual, emotional, recreational, and psychosocial needs are met/maintained individually.

About the campus: Crestwood Manor in Fremont is a skilled nursing facility serving individuals from a number of California communities. The salary range listed below represents the minimum and maximum base pay per hour at the time of posting. Final salary offered to the candidate selected for the position will be based on factors including but not limited to candidate's skills, experience, licensure, and program acuity.

Pay Range:

$25.50 - $26.50 USD
