Job summary

Humanim in Baltimore, MD seeks a Youth Transition Services Program Coordinator to coordinate services for program participants and support access to resources. You will help clients obtain needed services and build relationships with external partners to advance program goals.

The role reports to the Youth Transition Services Program Manager and requires strong coordination, communication, and time management skills to ensure timely service delivery.

Qualifications

  • Associate Degree in Human Services field required.
  • Reliable transportation required.
  • Valid Maryland Driver's license required.
  • Valid Automobile insurance required.
  • At-least one year of relevant experience.
  • Excellent interpersonal skills.
  • Excellent written and verbal communication skills.
  • Excellent time management and organizational skills - Meticulous to details.

The Youth Transition Services Program Coordinator, reports directly to the Youth Transition Services Program Manager, and is responsible for coordinating and ensuring the proper execution of services offered to participants in the program. The Program Coordinator assists clients in obtaining needed services or resources, and develops/maintains relationships with external partners.
