Job summary

Youth Advocate Programs, Inc. seeks a Program Coordinator to support the Philadelphia Program’s operations and case management, ensuring compliant, high-quality service delivery. Qualifications

  • Bachelor's degree required or equivalent experience in human services or related field.
  • Master's degree preferred for advanced responsibilities.
  • Experience in program operations, supervising staff, budgeting, or coordinating services desired.

Responsibilities

  • Provide operational and case management support for the Philadelphia Program and Program Director.
  • Serve as a key contact for youth, families, schools, and community stakeholders.

  • Maintain accurate case files, notes, and program documentation per agency standards.
  • Prepare and submit monthly and quarterly program reports.
  • Attend team meetings, case reviews, and trainings; support additional duties as needed.

Job description

Summary of the Position: Program Coordinator provides operational and case management support for the Philadelphia Program and Program Director to ensure effective, compliant, and high-quality service delivery. This role requires a strong understanding of program operations, contract compliance, participant intake and referral processes, documentation requirements, and community-based case management services.

The Program Coordinator serves as a key point of contact for youth, families, schools, and community stakeholders while helping to coordinate services and monitor participant progress toward individualized goals.

Support the Program Director in the day-to-day operation of the Philadelphia program.

Assist with program compliance, service delivery standards, participant intake, referrals, and case management activities.

Maintain collaborative relationships with local law enforcement agencies, schools, community organizations, and referral sources.

Communicate regularly with youth, parents/caregivers, school personnel, and other stakeholders to support service goals.

Serve as a liaison between youth, families, and community resources to facilitate engagement and access to services.

Monitor and document participants' behavioral, emotional, and social progress.

Maintain accurate and timely case files, progress notes, and case documentation in accordance with agency and contract requirements.

Prepare and submit monthly, quarterly, and other required program reports.

Attend and actively participate in internal team meetings, case reviews, trainings, and external stakeholder meetings

Support program initiatives and other administrative or service-related duties as assigned.

Qualifications/Requirements:

  • Bachelor's degree in Human Services, Social Work, Psychology, Criminal Justice, Education, or a related field; equivalent experience may be considered
  • Master's degree in a related field is preferred.
  • Minimum one year of experience supporting program operations, supervising staff, managing budgets, or coordinating services
  • Experience working with youth and families in community-based, school-based, or home-based settings
  • Strong organizational, communication, and problem-solving skills
  • Ability to manage multiple priorities in a fast-paced environment
  • Proficiency with Microsoft Office 365 applications, including Word, Excel, Outlook, and Teams; Experience with electronic health record (EHR) or case management systems is a plus.
  • Reliable transportation, valid driver's license, and current auto insurance required
  • Bilingual English/Spanish skills a plus
