Responsibilities

• Administers information and analytical systems to evaluate and enhance the quality of service provided to patients in the Community Care Program.
• Develops and implements information systems to track social work service activities including encounters, visits and unique Veterans served.
• Develops, implements and evaluates clinical guidelines and protocols to establish appropriate utilization of services.
• Supervises all social work staff in Homemaker Home Health Aide; Veteran Directed Care and Home-Based Primary Care Program and oversees the Caregiver Support program.
• Performs administrative (80%) and clinical (20%) services and manages staffing and leave requests.
• Provides feedback, evaluates performance, and implements personnel action as needed.
• Advises workers on work and admin matters; enforces safety regulations and training.
• Advances equal employment opportunity and nondiscriminatory practices in recruitment, promotion and development.
• Interviews veterans and families to assess needs and plan psychosocial interventions.
• Interprets VA treatment and benefit programs and makes clinical assessments and treatment options.
• Assesses high risk factors and determines need for services; makes independent professional decisions and educates trainees.
