Responsibilities

• Coordinate the activities and functions of designated programs.
• Analyze and evaluate program goals; ensure objectives are met within priorities, time, and funding.
• Serve as liaison between the district, community, health, and social service systems.
• Supervise, manage, and implement strategic program plans for the district Community Health Worker model.
