Job summary

Accessible Space Inc. in Minnesota is seeking a full-time Program Coordinator in Coon Rapids to support the Program Manager in delivering resident care plans, coordinating schedules, and ensuring accurate documentation.

The role involves providing quality personal care and health-related services, transporting residents as needed, and collaborating with the care team. A two-year degree in social services and at least two years of relevant experience are preferred.

Qualifications

• A two-year degree in a social services area of study or an equivalent combination of experience and education
• Minimum of two years of experience working with adults with physical disabilities and/or traumatic brain injuries in a rehabilitation or transitional long-term-care setting
• Ability to model excellent work behaviors and team-oriented problem solving
• Strong problem-solving skills and the ability to self-motivate and effectively manage time and shift priorities
• Ability to accompany and/or transport residents within the community

Responsibilities

• Assist the Program Manager in ensuring that the tasks on resident care plans are completed
• Assist with schedule changes
• Ensure completed documentation
• Provide quality personal care services and health-related services
