Job summary

Key Rehab is seeking a full-time Program Coordinator to join our team in Falls City, NE, managing therapy coordination in a SNF setting. You will serve as a liaison between Key Rehab and community staff, coordinate staffing and daily scheduling, and support clinicians to deliver high-quality care while meeting performance metrics.

Responsibilities

• In addition to providing therapy under their disciplinary scope, the Rehab Coordinator serves as the liaison between Key Rehab and community staff.
• This position is instrumental in minimizing administrative burden and providing support to allow clinicians to deliver therapy services.
• This position also coordinates staffing and daily scheduling to meet key performance indicators and expectations under the direction of a Regional Supervisor.

Key Rehab is looking for a full-time Program Coordinator (OT/OTA/PT/PTA/SLP) to join our team in providing excellent care in the Falls City, NE area in a SNF setting.

Education

• Required: Completion of an accredited PT, OT, or SLP program

Experience

• Preferred: Prior experience in long term care setting and service coordination

Licenses/Certificates

• Required: Current PT, OT, or SLP license to practice in the state(s) assigned
• Required: Certificate of Clinical Competence in Speech-Language Pathology (CCC-SLP)
• Required: CPR, as required by community
• Required: Valid driver license and car insurance as required by state law.
• Required: Other items as required by federal, state and local standards

• Experience with Geriatric Population: Prior experience working with elderly patients, demonstrating an understanding of their unique needs.
• Knowledge of Medicare Billing Regulations: Familiarity with current Medicare billing practices and guidelines.
