Job summary

Cambridge Health Alliance is seeking a Program Coordinator for the Healthy Lifestyle Program (HeLP) in Cambridge, MA. You will be the first point of contact for patients, supporting engagement through motivational interviewing and guiding them to the right level of care within a multidisciplinary team.

You will schedule group visits, manage patient registries, and coordinate care across departments, ensuring smooth handoffs and proactive re-engagement of patients who miss visits.

Qualifications

  • Education: Bachelor's degree preferred; equivalent experience may be considered.
  • Associate degree or high school diploma with substantial clinical administrative experience may be substituted.
  • Certification: Certified Health Coach desirable but not required.
  • 2+ years of administrative or coordination experience in a clinical/healthcare setting preferred.
  • Experience supporting weight management, chronic disease management, or pharmacy services is a plus.
  • Demonstrated scheduling, outreach, registry management, and care coordination skills required.
  • Fluency in English required; bilingual in Spanish, Portuguese, or Haitian Creole strongly preferred.

Responsibilities

  • Reach out to eligible patients, introduce them to the HeLP program, and support engagement using motivational interviewing.

Job Summary: The Healthy Lifestyle Program (HeLP) Program Coordinator plays a central role in connecting patients to the right level of care at the right time. HeLP is a multidisciplinary, team-based program at Cambridge Health Alliance supporting patients for weight management and wellbeing. This role is the first point of contact for many patients entering HeLP. The Program Coordinator reaches out to eligible patients, introduces them to the program, and uses motivational interviewing to support their engagement and readiness. They schedule patients into HeLP visits and group programming, orient them to what to expect, and when HeLP has a waitlist - connect patients to appropriate group visits or other departments in the interim. They maintain a registry of patients across the program and proactively re-engage patients who have cancelled, missed visits, or become lost to care. This position also provides coordination support for Wellness group visits through Primary Care, connecting eligible patients to the right group, orienting them to the format, and ensuring smooth handoffs between programs.

Qualifications and Requirements:
Education / Training: Bachelor's degree preferred; equivalent of 2–4 years of relevant experience will be considered
Associate's degree or high school diploma with substantial clinical administrative experience may be substituted
Certification: Certified Health Coach desirable but not required
Work Experience: Minimum 2 years of administrative or coordination experience in a clinical or healthcare setting preferred
Experience supporting weight management, chronic disease management, or pharmacy services is a plus
Demonstrated scheduling, outreach, registry management, and care coordination skills required.
Skills: Comfort working with patients managing complex conditions
Tech-savvy; able to coach patients and staff on telehealth tools and patient portal (MyChart) navigation
Proficiency with Epic, Google Suite, Excel, and video conferencing software
Clear written and verbal communicator; able to work independently and prioritize under pressure
Motivational Interviewing.
Fluency in English required. Bilingual proficiency in Spanish, Portuguese, or Haitian Creole strongly preferred; ability to work with a CHA interpreter when needed.
