Job summary

Passavant Memorial Homes in Pennsylvania is seeking a full-time Home and Community Services Program Coordinator. In this role, you will oversee the daily operations of the program, ensuring effective service coordination for individuals with disabilities.

Qualifications

• Master's degree plus 1 year experience with developmental disabilities.
• Bachelor's degree plus 2 years experience with developmental disabilities.
• Associate's degree or 60 credit hours plus 4 years experience.
• 5 years experience and strong leadership skills.

Responsibilities

• Implement Individual Plans as written.
• Report changes to Supports Coordinator.
• Attend IP/Interdisciplinary Team meetings.
• Ensure compliance with regulations and PMH policies.
• Provide emergency coverage as needed.

Education

Master’s degree with 1 year experience
