Job summary

Passavant Memorial Homes is seeking a full-time Home and Community Services Program Coordinator to support the Program Director in daily operations, ensuring IPs are implemented and accurate in Consumer Management. The role includes traveling throughout Lawrence County and reporting to the Home and Community Services Director.

The ideal candidate has a degree plus experience with developmental disabilities or extensive leadership experience, along with a PA driver’s license.

Qualifications

• Requires Master’s degree or higher with related work experience.
• Bachelor’s degree with two years’ experience in developmental disabilities.
• Associate’s degree with four years’ experience in developmental disabilities.

Responsibilities

• Ensure IPs are implemented as written.
• Report changes to Supports Coordinator.
• Attend IP/Interdisciplinary Team meetings as needed.
• Enter information in Consumer Management accurately.
• Provide emergency coverage and participate in on-call rotation.
