Description

Primary responsibilities of the Community Services Program Coordinator include supervision and support of Community Services Managers; monitoring Community Services billing; ensuring quality service provision; understanding budgetary considerations and funding sources; participating in the hiring process and staff development; coordination of services; and providing input to establish agency policies. The Community Services Coordinator must have the ability to follow the Agency leadership model. Facilitate new hire program orientation and serve as a certified investigator.

Responsibilities:

Coordinating Services for Individuals:

  • Facilitate the admission and provision of services for all Community Services program participants.

  • Promote agency philosophy to ensure high levels of individual care through community integration, individual choice, and development of individuals' competencies, while treating individuals with respect and dignity.

  • Review progress of individuals served through Community Services programs.

  • Serve as program liaison with families, Pennsylvania Office of Developmental Programs, County Administrative Entities, Supports Coordinators, and other service providers.

Professional Standards:

  • Adhere to and model the agency’s Code of Conduct.

  • Understand, support, and ensure adherence to agency policies and procedures.

  • Understand, support, and ensure adherence to federal, state, and county regulations and guidelines.

  • Maintain positive and effective relationships with program staff, participants, families, other service providers, Supports Coordinators, and the general community.

Program Management and Administration:

  • Lead and direct web based billing and electronic client record systems to ensure proper and timely documentation, reporting, and communication.

  • Monitor Community Services billing and review utilization.

  • Monitor Community Services programs for compliance with federal, state, and county regulations and guidelines.

  • Assist as necessary with the agency’s Summer Program.

Staff Supervision and Development:

  • Provide direct supervision, leadership, and mentoring to Community Services Managers and other assigned staff; give ongoing feedback and complete performance evaluations.

  • Support supervisory staff in the supervision of Home and Community Supports staff.

  • Participate in the hiring process; provide input into training needs.

  • Support and train supervisory staff in areas including, but not limited to, the implementation of and development of Individual Support Plans, provision of services for individuals, documentation of services.

  • Develop and present training to address staff development and program needs.

  • Encourage and promote a work environment that values the development and growth of others.

Agency Management and Administration:

  • Participate in development of agency policies and procedures.

  • Maintain certification as a Certified Investigator; conduct certified investigations as assigned.

  • Assist in the development of new programs; serve as liaison with the community and other agencies.

  • Provide input and assist in the monitoring of the agency budget and program expenditures.

  • Serve on Agency wide committees as needed.

Miscellaneous:

  • Responsible for the completion of other duties as assigned.

Hours:

  • 37.5 hours per week. Schedule encompasses varying days and hours, including periodic observation of all shifts. On-call as needed.

  • Workers may be Exposed to: Cold Heat Dust Mist Odors

  • Physical Requirements for the Position: ability to lift 50 pounds regularly, ability to respond quickly to sounds, ability to move safely over uneven terrain or in confined spaces, ability to see and respond to dangerous situations, ability to work in extreme weather, ability to wear personal protective gear correctly most of the day

Education and Experience

  • Must be 18 years of age.

  • Valid driver's license required.

  • Use of personal vehicle required.

  • Eligibility for Act 33/34 clearances.

  • Pre-employment drug screen required.

  • Minimum of two years supervisory/management experience and:

  • Master's degree plus at least one year experience working directly with individuals with developmental disabilities.

  • Bachelor's degree plus at least two years work experience with individuals with developmental disabilities.

  • Associate's degree or 60 credit hours plus four years experience working directly with individuals with developmental disabilities.
